DoD Brain Health Initiative Studies Effects of Combat

The Department of Defense is working to better understand brain health and combat. The DoD has expanded career-long cognitive assessments and weapon safety limits under its Warfighter Brain Health initiative to prevent brain injuries from routine blast shockwaves.

According to the Air Force official site, “The Defense Department’s Warfighter Brain Health Initiative aims to identify changes in cognitive performance to provide an opportunity for interventions to restore deficits and enhance cognition…”

Recruits across all branches take mandatory brain health tests when entering service, and those serving in combat specialties get priority screening and follow-up evaluations every five years.

Blast pressure from heavy weapons systems is among the main threats to service members. Firing heavy arms and explosive charges releases high-energy shockwaves during live-fire training or real-world combat. Weapons fire pressure waves can penetrate combat helmets and studies show repeated exposure can damage the brain and degrade dexterity and reaction times.

Combat Complicates Brain Health

According to AF.mil, that pressure is a major source of brain health complications.

“Blast overpressure occurs when weapons systems detonate, generating pressure waves that propagate through the air. These waves may have effects on brain health, impacting not only the service members operating the systems but also those in the surrounding area. For example, in training environments, drill instructors standing near trainees as they repeatedly fire certain weapons may experience frequent exposure, potentially increasing their risk of brain health impacts over time.”

The DoD’s Cognitive Monitoring Program serves as the DoD’s brain health evaluation tool. Troops complete computer-based assessments that evaluate ten functional domains, including working memory, attention, reaction speed, and visual processing. They also complete questionnaires tracking sleep problems, mood changes, pain levels, and head trauma history.

Without a review before live-fire training, clinicians cannot determine whether post-blast symptoms are caused by weapons fire or if the symptoms are the result of a pre-existing condition.

Blast Sensors and Weapons Safety Guidelines

The initiative uses “blast sensors” and weapon test data to establish training restrictions. Safety rules now include safe distances around heavy weapons, cap daily live-round counts, and related measures to prevent cumulative trauma.

The monitoring effort is an active program, not an experimental study requiring outside volunteers. Initial evaluations began in June 2024 for incoming troops, with assessments for active-duty personnel in high-risk occupational specialties a priority. The Department of Defense plans to have baseline assessments completed across the total force by the end of 2027.
