TRICARE Extended Care Health Option for EFMP Families
ECHO is not a standalone health plan. It’s a supplemental benefit for active duty family members with moderate to severe intellectual disabilities and serious physical disabilities. Conditions like Autism Spectrum Disorder also frequently qualify a dependent for these services.
Gaining access to ECHO is a multi-step process that requires proactive management from the service member. The first thing for military families to do is enroll in the Exceptional Family Member Program, or EFMP. This service-specific program ensures that the military considers the family’s medical and educational needs during the assignment process, matching EFMP families with assignments that can support them.
Once a family member is enrolled in EFMP, the sponsor registers for ECHO through their TRICARE regional contractor.
Benefits begin only once registration is finalized and the regional contractor has issued a pre-authorization for specific services. Families cannot pay for services out of pocket and expect reimbursement for care received before their ECHO start date. This is an important factor to consider when selecting coverage and care.
Benefits and ECHO Home Health Care
Covered services include:
- Rehabilitative services and specialized training.
- Durable medical equipment that exceeds standard TRICARE limits.
- Assistive technology and services, including interpreters or translators.
- Special education training for the use of assistive devices.
The ECHO Home Health Care benefit provides skilled nursing care and assistance from home health aides to beneficiaries who are homebound. It’s one of the most popular options, but these services must be provided by a TRICARE-authorized agency and are available in the United States, plus Guam, Puerto Rico, and the U.S. Virgin Islands.
ECHO offers respite care, allowing primary caregivers to take a break while a qualified professional steps in to monitor the beneficiary. In a typical month where other ECHO services are used, a family may qualify for up to 16 hours of respite care.

ECHO Costs
While there is no fee to enroll in ECHO, the program operates on a cost-sharing model based on the service member’s rank. Monthly copayments apply only during the months the family actually utilizes ECHO benefits. These payments scale with the sponsor’s pay grade, ranging from $25 for junior enlisted members to $250 for officers.
For most ECHO benefits, the combined coverage limit is $36,000 per family member per calendar year. It is important to note that the ECHO Home Health Care benefit is excluded from this cap, as it is managed under a separate fiscal limit based on what TRICARE pays for care in a skilled nursing facility within that specific geographic area.
Planning for Military Families
Because the program requires regional contractor involvement, a move from the East Coast to the West Coast (or an overseas assignment) requires a handoff to the family’s next regional contractor. The requirement for pre-authorization is an important safeguard, but it can also be a bottleneck. Why?
Because each product or service, from a specialized wheelchair to a session with a speech-language pathologist, must be approved in advance.
How to Register for ECHO
According to TRICARE, Active duty service members with family members must sign up for ECHO benefits via their branch of service’s Exceptional Family Member Program.
You must also register for ECHO with your TRICARE regional contractor. As mentioned above, there is no retroactive payment for services under ECHO. You cannot pay out of pocket for ECHO services and expect to be reimbursed after you have been approved for the program.
